New Patient Registration Online

Woodgrange Medical Practice is now offering online registration, this is open to all residents in Newham. 

To register please visit: - then enter your postcode and choose from the selection. You will be asked to fill in a form and submit your personal information so we can register you. Your registration will be processed within 2 working days. 

If you want to come in person to register, our receptionist will hand you a registration form which you will need to complete and return to us. 

You must check that your postcode is within our catchment area. 

You DO NOT have to show any ID or proof of address to register.

New Patient Registration on Form

If you currently reside within our Practice Boundary you can begin your registration as a 'New Patient' by completing the GMS1 registration form AND the New Patient Health Questionnaire.

Registering with our Practice - completing the GMS1 patient registration form

When filling in the registration form it is important that the form is fully completed to comply with the Health Authority requirements:

  • Previous surname (if any)
  • Town and country of birth
  • Home address with postcode
  • Telephone number
  • Previous address
  • Name and address of previous GP
  • If appropriate, date of arrival in the UK

  • Once the forms are completed and submitted please visit the surgery within seven working days.
  • Once the form has been received and actioned, you will be given an appointment with our Healthcare Assistant (HCA) to finalise your registration

Consultation with the Healthcare Assistant

Before a patient is accepted he/she must attend a consultation with our Healthcare Assistant; this is very helpful in providing us with brief information about your medical history. Afterwards you will then have a simple examination carried out by our Practice Nurse. This process will allow you to get to know our Practice and enable you to decide whether you wish to join our Practice.
Prior to booking a new registration appointment please bring in the following current documents:
  • One recent utility bill (which may include the current year’s council tax, electricity, water or gas)
  • Or a letter from your landlord or Housing Department to confirm your residency
  • If you were previously registered with another GP and are on repeat medication, please also bring the back-page of your prescription
Failure to provide documentation may result in the refusal of your registration
You can only see the doctor once this registration process has been completed
